Welcome to the Alaska Medicaid Electronic Health Record Incentive Program (EHR Incentive Program) State Level Registry, where providers, and other authorized users may obtain information about the EHR Incentive Program. This web site is designed to make it easier for you to interact with Alaska’s EHR Incentive Program. Like all online resources, we recognize that visitors to this web site are concerned about their privacy.

This Privacy Statement (“Statement”) describes the online privacy practices for Conduent websites maintained by Conduent, Inc. and its subsidiaries (“Conduent”).

The Alaska Medicaid Electronic Health Record Incentive Program State Level Registry discloses personally identifiable information only to authorized users including: fiscal agent and State of Alaska staff, State of Alaska contractors, providers who provide care to members, and others otherwise authorized by law.

Conduent recognizes that personally identifiable information collected by Conduent should be used only for the purpose for which it is collected. Except as authorized by law, such information will not be disclosed to other persons or organizations.

Conduent complies with all federal and state laws, regulations, policies, and rules, including (1) the Health Insurance Portability and Accountability Act of 1996 (HIPAA), Pub. L. 104-191, 110, codified principally at 42 U.S.C. ? 132d ? 1320d-6, and the HIPAA privacy and security regulations; and (2) the HIPAA Title II Administrative Simplification and Compliance Act provisions governing electronic transactions and code sets, security, unique identifiers and privacy, Pub. L. 107-105, codified principally at 45 C.F.R. Parts 160, 162, and 164.

Information Voluntarily Provided By You. Conduent may collect personal information about you when you use this website such as described above in “Personally Identifiable Information”. This web site collects voluntary information from you in several different ways, including through: e-mails, telephone calls, standard mail, a registration feature, an authentication feature, or other functions used by you on this web site.

How Personal Information May Be Used: The information we collect may be used to facilitate your use of our site and improve the operation of the site; respond to and fulfill any requests you make for services or other support from Conduent; communicate with you about the services available from Conduent.

Registration: In order to engage in certain online transactions and access restricted online information, providers will be required to register with the State Level Registry by submitting their Tax Payer Identification Number (TIN), and National Provider Identifier (NPI) as well as some additional demographic information about you, your organization and/or administrator.

Authentication: The authentication method used for granting authorized users access to secure content requires authorized persons to use their User Identification Number and Password.

When you visit our site we may collect information through the use of a “cookie”. A cookie is a small file that a website can send to your browser and then is stored on your computer. This type of technology is designed to tell us when you reenter our site, where you visit within our site, and the type of browser you use. You are not required to accept any cookie we might use, although some areas of a site may not be operational without the use of cookies.
